Swine Time Rides Application
We Promote a litter free environment.
Our 31st annual Swine Time Festival will be held on Saturday, Nov. 26, 2005.
We can accommodate two Space/Star/Moon walks, two Pony Rides, one Miniature Train
Ride, 2 Large Inflatable Slides, and not more than two Climbing Towers, plus other
approved rides.  No games of chance or games that resemble games of chance or
gambling will be accepted.

If you would like to participate, please complete the application below 
& send it along with a self addressed stamped envelope & a money order
made out to Climax Community Club for $200.00 per ride to 
Climax Community Club,

120 volt, 20 amp phase power supplied only.  Generators preferred. 

Entries will be accepted on a "first come" basis.  
If we are filled, your money order will be returned in your SASE and 
we will put your name on the waiting list in case of a cancellation.  
Please include your phone number so we can call you if there is a cancellation. 

Insurance is required for your entry.
Please include a copy of your declaration page that includes the amount of
insurance, company name & expiration date. (Min. $1,000,000. Ea. Occurrence,
Personal & ADV Injury, General Aggregate–$50,000. Fire Damage $5,000. Med.)

By signature above, Applicant accepts responsibility for any accident or mishap
associated with their Ride or other Entertainment entry and agrees to
hold the Climax Community Club and The Swine Time Festival harmless from any
litigation resulting from any accident or mishap associated with applicant’s
entry.  Swine Time is a family oriented event and will not permit any form of
gambling.  Vendors that set up any game of chance will be considered in violation
of the the Climax Community Club/Swine Time rules and will be asked to leave the
grounds with no refund of vendor fees or any form of arbitration.  Applicant
certifies understanding of rulesand will abide by the guidelines set by the
Climax Community Club/Swine Time Committee.
